The Bottom Line First
Grip strength is more than a measure of how hard you can squeeze. It is a quick, practical window into whole-body strength, physical function, and resilience.
Large observational studies have found that lower grip strength is associated with a higher risk of cardiovascular disease and premature death. That does not mean grip strength alone determines your future—or that squeezing a hand gripper will automatically add years to your life. It means grip is a useful signal worth tracking alongside walking ability, balance, blood pressure, activity, and other health measures.
The encouraging part is that grip strength can improve as part of a sensible full-body resistance program. Slow, controlled repetitions with bands, cables, dumbbells, or body weight can make that training approachable for adults over 50.
What the PURE Study Actually Found
The international PURE study followed nearly 140,000 adults in 17 countries. For every 5-kilogram decrease in grip strength, researchers observed:
- A 16% higher risk of death from any cause
- A 17% higher risk of cardiovascular death
- A 7% higher risk of heart attack
- A 9% higher risk of stroke
Grip strength was a stronger predictor of death in this cohort than systolic blood pressure. That is a striking association, but it is important to keep the language accurate: this was an observational study. It shows that lower grip and poorer outcomes travel together; it does not prove that grip weakness directly causes those outcomes.
The UK Biobank Adds More Evidence
A separate UK Biobank analysis included more than 500,000 adults ages 40 to 69 and followed them for an average of about seven years. Each 5-kilogram reduction in grip strength was associated with a 19% higher risk of cardiovascular death in women and a 22% higher risk in men.
Taken together, these studies support using grip strength as a low-cost health marker. Grip reflects more than the hand and forearm. It can also capture the effects of physical activity, muscle mass, nervous-system function, nutrition, illness, and overall frailty.
There Is No Single Magic Grip Number
You may see fitness targets such as 40 kilograms for men over 50 and 25 kilograms for women, but those are better treated as aspirational gym goals, not universal medical cutoffs.
The European Working Group on Sarcopenia in Older People uses grip strength below approximately 27 kilograms for men and 16 kilograms for women as a clinical warning sign of low muscle strength. Even those values must be interpreted in context. Results vary with age, sex, body size, hand dominance, pain, equipment, and testing position.
Your most useful benchmark is often your own trend under consistent testing conditions.
How to Measure Grip Strength at Home
A hand dynamometer provides the clearest number. To make repeat tests more meaningful:
1. Use the same device and handle setting each time. 2. Sit or stand in the same position with your elbow comfortably bent. 3. Squeeze firmly without holding your breath. 4. Perform up to three attempts per hand with a short rest between efforts. 5. Record your best result—or your average—but use the same method every time. 6. Retest every four to six weeks rather than every day.
A sudden or unexplained decline deserves attention, especially if it appears with fatigue, weight loss, pain, numbness, or difficulty with daily tasks. Discuss it with your healthcare professional.
What About the 20-Second Dead-Hang Test?
Dead hangs are popular online as a test of grip and shoulder endurance. Holding for 20 to 30 seconds can be a reasonable fitness challenge for someone whose shoulders, elbows, and wrists tolerate hanging, but it is not a validated longevity test.
Skip full hangs if you have shoulder instability, rotator-cuff pain, elbow or wrist problems, numbness, or recent upper-body surgery. Safer ways to train grip include:
- Suitcase or farmer carries
- Holding light dumbbells at your sides
- Towel squeezes
- Resistance-band rows
- Supported hangs with some weight remaining on your feet
The right test is the one you can perform safely and repeat consistently.
Slow-Tempo Training: Useful, but Not Magic
Time under tension means controlling how long your muscles work during each repetition. A simple approach is to lower or return the resistance over three seconds, pause briefly, and complete the lifting phase with smooth intent.
Research does not show that one exact tempo is uniquely superior for muscle growth. A systematic review found similar hypertrophy across repetition durations ranging from roughly 0.5 to 8 seconds, while extremely slow repetitions lasting more than 10 seconds may be less effective.
So why use a three- or four-second lowering phase?
- It encourages better control and body awareness.
- It reduces momentum and makes lighter resistance feel challenging.
- It helps you notice discomfort before it becomes a problem.
- It works especially well with bands and cables, which maintain tension through much of the movement.
Tempo is a coaching tool—not a biological loophole. Progress still comes from consistency, adequate effort, recovery, and gradually increasing the challenge.
A Joint-Friendly 15-Minute Strength and Grip Session
Perform this routine two or three times per week on nonconsecutive days. Choose resistance that leaves about two or three good repetitions in reserve.
1. Resistance-Band Row
Perform 2 to 3 sets of 8 to 12 repetitions. Pull smoothly, then take three seconds to return your arms to the starting position.
2. Sit-to-Stand
Perform 2 sets of 8 to 12 repetitions. Stand with control, then lower to the chair over three seconds. Use your hands for support if needed.
3. Band Chest Press
Perform 2 sets of 8 to 12 repetitions. Press forward smoothly and take three seconds to return.
4. Suitcase Carry
Hold one dumbbell or kettlebell at your side and walk for 20 to 30 seconds. Stay tall without leaning. Complete 2 to 3 rounds per side.
5. Towel Squeeze or Supported Hang
For a towel squeeze, hold a rolled towel and squeeze for 10 to 20 seconds, then relax. Complete 2 to 3 rounds. If hanging is comfortable and appropriate for you, keep your feet supported and use only as much body weight as your joints tolerate.
When you reach the top of the repetition or time range with stable form, add a small amount of resistance or a few seconds. Change only one variable at a time.
Build the Body Behind the Number
Grip strength is valuable because it reflects the broader system. The goal is not to become excellent at a single hand test while ignoring the rest of your body. Train your legs, hips, back, chest, shoulders, core, and grip together. Walk regularly, eat enough to support recovery, and protect your sleep.
That is how a number on a dynamometer becomes something more meaningful: easier grocery carries, steadier movement, stronger hands, and more confidence in daily life.
